Don't know if it's been posted or not (it's not in the OP or Quick Summary), but here's the phone number for:

This information is really buried in their CONTACT US section (you need to search for it and jump through a few more hoops). It seems like Sam's Club would rather you contact their local stores (which don't deal with online orders) than corporate directly...
