If you want a premium channel for $9.99/month, or if you're paying more than $10/month for each premium channel (whether HD or not, it doesn't matter), here's a simple deal that'll save you a little each month, at least on HBO, Showtime, and Cinemax (maybe Starz, etc... I didn't check):

Call and cancel your premium channels (if they ask why, feel free to say "don't worry about it, it's a long story"

After one or two days, once your On-Demand access to your previous premium channels is updated and you're no longer able to access them for free, try to order any show (movie, series, whatever) from your desired premium channel.

***Do NOT try to order through On-Demand premium HD section, or this won't work. You must order thru regular (non-HD) On-Demand!***

When a new On-Demand window pops up and says "PACKAGE NOTICE: The program you have requested is only available as part of a package", choose the option "Package Information". It'll then give you the option to buy (and it should show a price of $9.99 at the upper right corner of the screen)! Just choose to buy the so-called package and you'll have that premium channel at $9.99/month. It may take a couple days to activate On-Demand access to the HD section after buying this package, but it all works perfectly a day or two later.

Let me know how you guys do!

That's what it says if you get to the Premium section through the HD section, and it won't work that way. Did you go through the regular (non-HD) On-Demand section? There was no option to view "Package Information"?
