The wording on the rebate form is a little odd. It asks for a copy of the cut UPC code.
ETA: From the rebate form: "Rebate value will not exceed purchase price."

As with previous AMZN money maker rebates, if your rebate check comes and it's shorted, just contact AMZN and tell them you were shorted, and send them a PDF of the page showing it would be -$6.01 AR. They'll credit you the diff.
