The fine print suggests that we can:

If one happily would otherwise pay $10+ for movie tickets, then this is worth it to you. But if one doesn't go to movies often, or goes to matinees instead or uses other means for cheap tickets, then there are other Netflix signup offers that might be better, like the occasional $15 eBay bucks offer. Not knocking this deal, just offering another perspective.

I used a different email address and credit card last time I signed on a special offer with no problems. Not sure if you have to change credit card but you will have to use a different email.

Users over on the "other" forum reported to get the code within 20 minutes of streaming 3 Netflix shows. I think it's more like "you have 4 weeks to stream 3 shows, and we'll give our self a week just in case"
Others have also reported using multiple emails/CCs works too.
Although others have reported waiting >12hrs with no codes.

How did they get your address? They never ask for it unless you sign up for the dvd out option. I am guessing if you got denied the signup offer you used a CC that you used, or tried to use before.
