Can anyone send me or link me to the $50 rebate form? I didn't get it before the deal ended and the link is dead along with any reference to it on their webpage.

Done both. Nothing on rebate page, and chat with customer support says they don't know what I'm talking about. The agent talked to the supervisor, but they still didn't find it. Right now, I'm awaiting a phone call to tell me what they decide. I had issues last year with a HP ZR24w rebate that they rejected citing that the promo never existed. Took two months to finally get it, but that time I had the form and this time I have no evidence the promo existed.

Thanks for the link. At least I was able to see the promo information. But the links to the rebate form just toss you back into the new ad for the monitor with no mention of the rebate. If I can't get a form, I can at least send them a snapshot of the ad with the rebate discussion. If anyone has a form though I'd still appreciate it. Can't believe they hide the forms as soon as the ad ends.

I found the original rebate form on an expired e.bay page:
