I was sure someone should have posted this already, but I can't find anything on the subject.
There is a Sprint PCS / Nextel rebate form on the inside of select boxes of Kellogg products, that may be used to receive $75 in addition to any other rebates. New activation on 2 year contract is required and phone must be priced $299 or above. I'm not sure that it's really a good deal for everyone, but if you, for some reason, are buying a phone that's not free, why not get extra $75 back? (Well I'm a T-Mobile addict, I don't care.)
Edit: It turns out that the form may be found on the Kellogg web site, see below.
I'm going to write all I know about this rebate in Quick Summary, then anyone who knows more can add/modify that info.
PS A few fatwalleters pm'd me asking for the form. I'm not sure if you still want it, or will just download one. If you do, pls pm me again.

Sprint PCS / Nextel $75 rebate form, available on the inside of Kellogg packages marked with "SAVE UP TO $225 ON SPRINT NEXTEL". (Mine was on Nutri-Grain bars, but it should be on other products too.)
The form may be seen on the Kellogg's website here. (Thanks goes to bubbarwanda for digging it out.) It's not clear if the printout may be used in lieu of the actual form.
Here is the summary of conditions, as stated on the form and instructions next to it.
Customers can redeem either or both rebates. (That refers to "up to $150 rebates" offered elsewhere.)Offer begins 7/2/06. Requires purchase by 9/30/07 and activation and postmark by 10/15/07 on a phone $299 or higher.Service plan: $36 activation required.Clip out original purchased phone label with serial number from the product box. (It appears that ordinary Sprint rebates require only copy of the label.)Rebate mailing address: Sprint Kellogg Rebate Offer Dept./Promo #06-99066 P.O. Box 26622 Shawnee Mission, KS 66225-6622.
I just purchased a new phone ($299 w/ a $100 rebate from audiovox) with a 2 year renewal contract with Sprint. I did so a couple weeks ago. Could you please post the terms and conditions (specifically, the date which this promotion started)? Thanks
Pulled this off the Internet on some site advertising the rebate information:

Sprint Kellogg Rebate Offer Details:

Rebate forms must be postmarked by 10/15/2007 to qualify for the additional $75 Mail-In rebate. The Sprint Kellogg Rebate offer cannot be combined with other Sprint National or Exclusive Handset overlays. Customer must maintain an active service for 30 consecutive days, allow 6-14 weeks to receive the rebate and review full terms and conditions for the offer as well as check rebate status by visiting www.sprint.com/kellogg. *Only handsets with a minimum SRP of $299.99 qualify for this offer. A list of specific handsets will be periodically posted in the AR Program Update.
Do you mean MSRP of $299.99? Most phones are now offered with instant rebate, say $299 less $150 instant rebate and you pay $149 out of pocket for a phone with a new service. So in this way it qualifies for the rebate?
Also, it seems like only selective specific handsets are in the rebate program.

I guess it's worth a shot to send a copy of the UPC to this rebate. I'm gonna check out the HEB tomorrow...HEB?
