DOCKERS® FREE HOTEL BREAK OFFER TERMS AND CONDITIONS
Offer date: 12:01 a.m. (EST) 6/7/07 – 11:59 p.m. (EST) 6/16/07. Redemption requests must be postmarked by 11:59 P.M. 7/1416/07. Must be 18 years of age and a legal resident of the 50 United States or District of Columbia. Void in Puerto Rico, Guam and where prohibited. Purchase $100.00 or more (pre-tax amount after application of any discounts) in qualifying men's Dockers® apparel (excludes Luggage, Home and Shoes) at participating Kohl's Department Stores® and receive a Free Hotel Break booklet by mail good for a free two (2) night hotel stay for up to two (2) people and two (2) free entrees (excluding appetizers, beverages and gratuity) for up to two (2) people at a participating hotel and restaurant. Multiple, same day sales receipts indicating a qualifying Dockers® purchase(s) may be combined to achieve the $100.00 minimum purchase requirement. To receive your Free Hotel Break booklet, complete this form and mail it, along with your sales receipt(s) (only originals accepted) with required purchases circled to TLC Marketing, C/O Dockers®/Kohl's, 1 Faneuil Hall Marketplace, 4th Floor, Boston, MA 02109, postmarked no later than July 16, 2007. If internet access is unavailable please call the Dockers® Concierge at 866-377-4084 Monday thru Friday 8:30AM to 8:30 PM EST. This offer is subject to the terms and conditions set forth in the booking materials. Hotel taxes and other fees are excluded and where applicable, are payable upon booking. The Free Hotel Break offer is valid until 7/16/08, and the Free Hotel Break offer must be booked not later than thirty (30) days prior to 07/16/08The Free Hotel Break offer is valid until 7/31/08. All bookings must be made by this date. Reservations can be accepted for up to a 6 month period after this date if booked by 7/31/08. The booking instructions (how to book) and all other information will be available on the Free Hotel Break booklet and booking form you receive once qualified. Hotel stay must be booked through TLC a minimum of 30 days before desired hotel stay date. Consumers will not be responsible or charged for any shipping or handling fees to receive this offer. Requests that do not meet the terms and conditions of this offer will be denied. Limit one (1) request per person, per household. Requests from P.O. boxes, dealers, businesses, groups or organizations will not be honored or acknowledged. This offer cannot be combined with any other offer. Free Two Night Hotel Stay and Dinner Terms and Conditions
1. The "Hotel Stay" offer is to obtain two free nights hotel accommodation for up to 2 people sharing a room. The person in possession of the voucher can book a hotel stay at any participating location in the United States.
2. The "Dinner" offer is to obtain up to 2 free entrees (excluding appetizers, beverages, tips and special orders) for 2 people sharing a meal. The free dinner offer is only valid for redemption during the same time period as the "Hotel Stay" booking dates. The reservation process varies from hotel to hotel and will be detailed at the time of booking.
3. This offer is subject to promotional availability.
4. Room tax and other hotel fees are not included and if applicable may be payable at the time of booking. A deposit or payment in advance may be required.
5. This offer has no monetary value is not for resale and cannot be redeemed in whole or part for cash.
6. The promoter and TLC accept no responsibility financial or otherwise for the misuse of this voucher, and under no circumstances will they be responsible or liable to compensate the purchaser or bearer of this voucher, or accept any liability for (a) any non-acceptance by a venue of this voucher or (b) any inability by the bearer to use this voucher properly or at all; or (c) the lack of quality or any other aspect of any service which is or should be provided at any venue, or (d) the contents, accuracy or use of either this voucher or the venue listing, nor will they be liable for any personal loss, property loss, illness or injury occurring at any venue.
7. Photocopied vouchers will not be accepted and the offer is not valid in conjunction with any other offer including but not limited to; TLC offers and hotel discount programs that are offered exclusively by specific hotels or hotel chains that require the consumer to make the hotel booking directly with the hotel.
8. The promoter and TLC Marketing may cancel this promotion at anytime, without notice.
9. To qualify all reservations must be made through TLC Concierge Service.
10. Any cancellations or changes to reservations after confirmation by TLC must be made with the hotel directly. Customers are solely responsible for any charges and/or fees arising from changes made directly with the hotel. Once confirmed, Customers who do not make their scheduled reservation are subject to charges and/or cancellation or room fees if applicable and vouchers will no longer be valid for use.
11. This program is void where prohibited by law. To be eligible to use your hotel stay Voucher you must be over 18 years of age (some hotels may require an age minimum of 21 years).
12. A minimum of 30 days notice before travel is required.
13. This voucher is valid until 7/31/08. All bookings must be completed by this date. Reservations can be accepted for up to a 6 month period after this date if booked by 7/31/08.
14. TLC Marketing may substitute another reward of equal or greater value if promotional availability runs out.
