3M insert is some ad for 3M products. Staples will insert such things in your package.... September flyer, 3M insert, Bose insert etc etc

It is free after rebate. I just got the Ready Boost one this morning in Manhattan (42 and 3rd has them in stock) and it was $24.99 and the rebate form printed out from the register.
