I visited the page with the given link: It has a link to download the rebate form /and/ another link to close the window.
I read the complete page and nowhere it talks about the $5 review bonus

But what if you bought a combination of 4GB and 8GB sticks? The rebate form says the Offer number must be written into the envelope address, but there is a different offer number for each item.
Does that mean you should write both offer #s on the envelope, or submit two different enveolopes?

Well, I stopped by my local Frys this AM and picked up 3 of the 8GB sticks because they had them on sale for $50 each (the 4GB sticks were also priced at $50 so I didn't get any of those) and I can confirm that the product number is MSDP8GB-40-664 (Rev B) with a UPC # of 6-50590-14603-1, made in Japan.
I had wanted to also get some of the 4GB sticks, so I ordered them online along with another 3 of the 8GB sticks since I thought I might need to have them all on the same receipt to submit the rebate. I'll return the 3 8GB sticks I bought in-store when I pick up the ones I ordered online and chose in-store pickup for free delivery.

Just got off the phone with the Lexar rebate center (live person).
1) She confirmed that purchases from Fry's are valid for the rebate
2) I asked to confirm that I could submit a rebate for 3 of the 8GB sicks and 3 of the 4GB sticks. At first she said that I could only submit up to a total of 3 rebates, but only one rebate per product. Since this clearly contradicted my understanding of the words "Limite (3) three rebates per product per address" which to me means I can get 3 8GB rebates AND 3 4GB rebates with the same address. She put me on hold to check, came back and said she was mistaken and that I was correct.
3) She said I would need to submit the rebates for each product (4GB and 8GB) separately (which supports my reading of the rebate form which says the Offer # must be included in the address and each product has a different offer #). She also commented that I might want to submit each rebate separately since mutliple rebates in the same envelope sometimes get mixed up and they only record one of the rebates. I'm still trying to decide if I want to do this because I've had mixed results with past rebates (not Lexar).
