for those of you that purchased this item without knowing you had to print out the rebate form, here is the link to the rebate form in pdf format, because when i logged on the next day to find out why a rebate didn't come with my headset, i realized it had to be printed, and then tried finding the link, only to realize that the rebate had been completely removed from the item page, and then called customer service, and even they were stumped, then the rep realized i was looking at a different seller's item, and redirected me to the right page, and showed me how to find the form, btw, if you're wondering, this headset seems ok for the price, it's not as loud as i'd like, and picks up on other people talking in the room, or loud background noises, it doesn't pick up things like typing on your keyboard, or a sink running, but it will pick up things at the same volume as someone talking, it works with my t mobile g1, and the battery life seems decent, for my first headset, it works well

I am noticing that the rebate form says to send them in separate ( I ordered 2)
everyone else doing that ????

the rebate form states only copy of "original store-identified receipt" from Amazon, it mean package slip or online invoice?
damn, there is no package slip in my package box. I planned to send the copies of both.
