Anyone can make a form in Google Docs and get a similar URL. I created a similar form in Google Docs that has a similar address:

and all the forms created have the TOS at the bottom of them.

The forms input into an Excel-type spreadsheet in Google Docs, I would think that Google is a big enough company that they would not be using Docs to gather information.

I suggest clicking the "Report Abuse" at the bottom of the original link form and submit it for phishing.
