was a little more to it at the bottom asked for a link to the poster you made ... i searched for it and made a link with it but it didnt seem to work .. i still pasted it in the box anyways hope it comes .. thanks
I don't trust this... why would google put this in Google Docs? Anyone can make a form in Google Docs and get a similar URL and all the forms created have the TOS at the bottom of them.
The forms input into an Excel-type spreadsheet in Google Docs, I would think that Google is a big enough company that they would not be using Docs to gather information.

I suggest clicking the "Report Abuse" at the bottom of the original link form and submit it for phishing.

It comes from an "https" (secure http) address. I don't believe that you can make one of those in Google docs.
