Well, I thought it quite odd that they would ask for your previous address if moved in the last 12 months. The only time you get that is from people checking or offering to give you credit, in my experience. But the domain name was registerd many years ago, so who knows, this could be legit.

When you fill out the online form make sure you indicate that Camel is your Brand. If you have trouble getting verified just call them up.
