It seems to me that it's maybe going to have a list of the emails that it sent invitation's too, if your email isn't on the list, it doesn't seem as it will let you take the survey.
I will add this though, when you make a purchase in one of the stores, they randomly put a survey code on the bottom of the receipt (It's not this site, I've done it before and it looks nothing like this site) but it will be for 5 dollars off a appetizer. Which most are under $5, so you can get it for free, sort of, since you still have to make a purchase to get the code but.....

You can also sign up for the Dragon Club through the website. $5 off a purchase $10 or more.

It says right in the original email that it is a personalized email good only for the recipient. The survey was really buggy anyway, after my third time being kicked off, I stopped trying.
If you want to periodically receive these surveys, just sign up for the Pick Up Stix Dragon Club (it's free) and while you are at it sign up with a few different e-mail addresses (different members of your family or friends, etc.) and then once every few months when they have new menu offering ideas they will send everyone on their Dragon Club list a survey and if you complete it you can print out a personalized $5 rewards voucher that is usually good for about three to four weeks and only good at one restaurant (the one you chose when you began the survey). Usually they have a lunch special and $5 pays for the majority of your meal.
Their current survey is now closed:
Thank you for visiting our survey site. Our survey is now closed; due to high volume we have completed the survey early. We look forward to your input on our next survey.
