http://icecreamsettlement.com/Home.aspx has claim forms for Class Members who purchased Ben & Jerry’s and Breyer’s Ice Cream. Class Members are entitled to receive $2 per unit of ice cream purchased, up to a maximum of 10 claims per household. The deadline to submit a claim is August 30, 2012.

Sorry, don't keep my Food Lion receipt for lettuce and ice-cream on file, but.... "Any individual Claim for reimbursement for more than three units will also require documentation in the form of an authentic Proof of Purchase such as receipts or UPC codes."
There are separate claim forms for Breyer's and B&J's.
Hazard to guess whether the "three units" is per defendant?(would mean $12) or cumulative of claims filed against both?(would mean $6)

welcome to FW, OP. Might help others if you added "Class Action Suit forms available" to the title.
My initial thought was that this thread would be more about Breyers no longer making real ice cream. Second thought was coupons.

Says up to 10 claims per household. I would guess you can put them all in one envelope. Do this once each for both Breyer's and Ben and Jerry's. This could add up.
