Heineken $10 MIR to buy $20 worth of premium cuts of fresh beef, marinades, seasonings, salty snacks and ice AND one 12 pack or equivalent or one DraughtKeg of Heineken or Heineken Light. NBPR for NC or VA. Send originals (no copies) this form and dated cash register receipt(s). Purchase between 5-1 and 7-31-09. Request to be postmarked my 8-14 and received by 8-21-09. I do have extra forms to trade.
This form does not list any BPR states the MIR is valid for. There is no (usuall) toll free telephone number. Address is Heineken Block Party Nights Offer, Dept. #8059, PO Box 3002, Rogers MN 55374-3002. Does anyone know what the BPR states are for this MIR? Thanks.
Too bad the rebates were sent back to me after you updated your post saying they were valid in my state. Heineken says it's only good in VA and NC. Here's the email I received:

You need the Heineken forms that the premium cheese, etc goes to P.O. BOX 8019 and the beef, etc. to P.O. Box 8021. Those are the Heineken forms that are good in MO and do not require a beer purchase. You just need to find those forms!
That you chose to ignore my posts and PM sent to you regarding this problem says a lot, don't you think? I've posted to your attention in several threads and sent at least three PMs. Horrible follow up customer service! (added 7/4/09).

I, too, am still waiting for my $10.00 rebate from Heineken. I do live in VA and sent in all the required proofs of purchase 10 weeks ago. I cannot find an e-mail address in which to complain to the Company and request my rebate. Can you help me?
