Red Hook $25 (total) MIR's. This form is 5 different $5 rebates on 1 page. Purchase $5 or more for Beef for a $5 rebate. $5 for Chicken, $5 for Seafood, $5 for Spices and $5 for Pasta. Each is a different offer so one may do all 5 (probably in 5 different envelopes, even though they all go to the exact same address) There is NO "start date" on the forms. Purchase before 12-31-09. Postmarked by 1-15-10. NBPR states; AL, AR, CT, HI, KY, MA, ME, MN, MO, NC, NJ, NY, SD, UT, VA and WV. BPR states; AK, AZ, CA, CO, DC, DE, FL, GA, IA, ID, IL, KS, MD, MI, MS, MT, ND, NE, NH, NM, NV, OH, OK, PA, SC, TN, VT, WA, WI, and WY require the $5 (food) purchase per rebate + one 12-pack or two 6-packs of Redhook.
