Purchase between 81 and 10-4-09. Request received by 10-19-09. Send originals (no copies) this form and dated cash register receipt(s). Valid for AL, AR, CT, HI, KY, MA, ME, MN, MO, NC, NJ, NY, RI, SD, UT, VA and WV. I have extra forms to trade. Thanks.

I second that, babswhyit. I have had 14 trades with Alan and all have been seamless. His shopping is super fast and he always throws in extras. If anyone reads his "main" post they'll see all the positive feedback he has received.
