Request received by 10-19-09. Send originals (no copies) this form and dated cash register receipt(s). Valid for AL, AR, CT, HI, KY, MA, ME, MN, MO, NC, NJ, NY, RI, SD, UT, VA and WV. I have extra forms to trade.
