When I printed the PDF it said the expiration date was in January of 2009.

Bottom of PDF says: "Submission must be received by 1/30/2009."

That is probably what Shayrose is seeing.

Looks like a typo to me, based on the other dates on the form.

The download link is directly from actII.com, which is registered to conagra foods. Judging from that, I would say it is more than likely legit.
