Here is yet just another example why I NEVER throw away grocery store receipts. Came accross this form today but the start date is 3-30-09!!!
Coors $5 MIR to buy $20 worth of Salty Snacks, Ice, Water, and/or Soda (any brand, any size). Purchase between 3-30 and 12-31-09. Received by 1-29-10. Residents of AL, KY, ME, NC, VA, VT, and WV. NBPR. Send originals (no copies) this form and dated cash register receipt(s). I do have extra forms to trade.
