Budweiser $15 MIR for TV purchase to exceed $399.00. NBPR. Send originals (no copies) this form and dated cash register receipt. Get a duplicate receipt for this or your warranty records. Purchase between 10-4 and 11-20-09. Postmarked by 12-4-09. Valid for AL, AR, KY, MO, NC, UT, VA and WV. I do have estra forms to trade. Thanks.
