Does anyone know if you can use the rebate form beneath the Newegg link for Amazon? The shipping for Newegg is $4.99 and they charge tax as well.

Why not just use the rebate form beneath the Amazon link in the OP for Amazon?

I made an assumption. OP did it right and used the same rebate form for both. On Amazon itself has a rebate form that expired on 6/30. That's where I made the assumption that it was the same rebate listed below Amazon's link.
