Costco has TurboTax Deluxe for $4.99+tax after 2 MIRs and 10 dollars off coupon:

TurboTax Deluxe: $34.99

Costco Turbotax MIR: $10
Intuit manufacture MIR: $10
Costco club member $10 off coupon from coupon book: $10

Total = $4.99 + tax

This is a good deal for those who want to buy the turbotax alone without another combine deals(Quicken, Norton antivirus etc..)

Instuit's mfg rebate explicitly excludes Costco and Sams Club. In addition. the Intuit P/N and UPC for the Costco version of TT deluxe is different that the standard retail TT Deluxe version. Even at $14.99 AR with nothing else to buy this is still a good price especially if you do not want to fool with the multi-product deals other stores are offering.

Ask for it at the Costco service desk. If they are out of the coupon books you simply need to mention the $10 coupon to the cashier on your way out and they will take it off without having an actual coupon.

Any mention of Free State version with the Costco $14 TT offer ?

Actually i checked the Intuit rebate center for the $10 rebate(offer number 03-63884) They only mention those purchased from Wal-Mart, Sam's, and Target are not eligible for the dollars-off mail-in product rebate.
So is the rebate form inside the box specifically mention Costco? Or there is actually no rebate form inside the box at all?

There is no $10 in-box rebate form with the Costco version. As posted earlier, The TT Deluxe UPC and Intuit P/N on the box is also different. You also call the Intuit rebate center and they will tell you Sams, Costco, and a few others are excluded for their offer and to use the rebate forms you recevied from those retailers. The best deal you are going to get at Costco is $14.99 after a $10 rebate plus sales rax on $24.99 if applicable.
