Upgrade rebate is not listed at CC website, but seems good to any Nero 7 Ultra Edition purchase.

Requires cover page of manual, so OEM versions won't work.

No, its more like YMMV. Include a pre price-match receipt with rebate. For me, in the last 10 PM/ Rebate deals at CC (in the last 3 months), I have never had any issue with any rebate, they all came for the full advertised amount with no follow-up.

In store pickup isn't available. But if the online order receipt doesn't say anything about rebate being offerred then credit card BVG can be done.

Not entirely sure, but judging from the dates (10/9/05 - 10/8/06), I would guess it is the same upgrade rebate.
