In-stock orders cannot be modified or canceled! That said, here are the four easy steps:
1) Specify that you have a broken Canon XYZ camera and would like to know what the loyalty upgrade options are. They will try to transfer you to a technician, but you can tell them you just want to know the prices first and the rep can give them to you. (Update 9/13/12 - Rep said no need to transfer to tech, can just take your info and serial and process)
2) Once you are ready, call again. This time, you will get transfered to a technician, who will ask you to desribe the issues. They would verify that 1) it's broken* and 2) it's out of warranty (just say you've had it for a over a year). Then, he would ask for the broken camera's serial number, your name, phone, address and email.
(*TIP: if you specify water damage, e.g., it won't turn on after it got wet, they will accept it as broken for sure)
3) The technician will give you the prices as well, then transfer you back to the loyalty program rep, who will take your name, phone, address again as well as your credit card. You will get the order number.

4) Canon will ship you the replacement camera with FedEx signature required, along with a prepaid FedEx label and a box for you to ship your broken camera to them. You must ship it to them within 10 business days to avoid unreturned product fee. You only need to ship the camera body, so you can keep all accesories (batteries, charger, straps, etc.). Thanks OP. For my daughter's busted SD800-IS I was offered a refurb G9 for $250. Other things they offered was SD850 IS $150 and the XT for $275. No interest in the latter, I am still thinking about the former. I asked if there is an expiration date on the offer and was told that there is not

CZConsidering the only decent deals so far are for fairly decent cameras, and even then aren't massive, probably just not worth it. I am half way tempted to scam a serial number and see what offering a digital rebel would get though.
Though actually a known problem with some of the canon digital cameras http://www.usa.canon.com/consumer/controller?act=PgComSmModDisplayAct&keycode=2112&fcategoryid=226&modelid=11154 is that the serial number wears off, so you could see what you could get without having the camera yet, but I am just too lazy.
Ed: actually since the info is in the exif you can go to flickr search by camera select a camera of the sort you were thinking, and click more properties on one of the images (may take a few tries find one that displays exif). Then you would at leaat know if it was worth it... but far far too much work for little old me
