All Amazon.com Points will be doubled for the first 90 days from your enrollment date in the program. You do not earn Amazon.com Points on balance transfers, cash advances, cash-like charges such as travelers checks, foreign currency, and money orders, any checks that are used to access your account, overdraft advances, interest, unauthorized or fraudulent charges, or fees of any kind, including fees for products that protect or insure the balances of your account.

It took me 4-5 calls before I got the "upgrade" or more precisely a "product trade." The problem was that the people on the other line were not aware of this promotion and they kept telling me to call the "Rewards" department (888-827-9369).
Every time I called I was asked for a promotion code but there is no promotion code. The lady on the phone, who I talked to, had to search for this promotion and then do a "product trade" once she found it. It took her two tries to find it. (If they can't find it it does not mean that you don't qualify, they just need to change their search criteria.)

During my last phone call I just said that it would be silly for me to have to apply for a 2nd card to get this promotion and shortly after that the lady found this offer and traded my old one for the new one.
All I can add is be patient and keep on trying.
