Tmobile 15% discount - Members of LifeTime Family Fitness

I check and no one has posted this, I think its a great deal for all the people who are members of Lifetime Family Fitness.

Existing T-Mobile Customers who would like the 15% discount -
Click here to download the Migration Form and review important migration information.
Review the Instructions section of the form for program details and expectations.
Complete the bottom two sections of the form and e-mail it to
lifetimemembersmigrations@T-Mobile.com. Be prepared to submit your Life Time Fitness Membership number, and first and last name as it appears on your membership card.

NOTE: T-Mobile customers who applied for the discount at 10% level, do not need to submit another request. The discount will automatically be adjusted to the 15% at the start of your next bill cycle.

To sign up for NEW T-Mobile service or add a new line to an existing account -
Call T-Mobile Member Activations at 1-866-464-8662 (select option 3) to order new service or click REDEEM ONLINE to view all member offers or place your order on-line. Please have your Life Time Membership number ready.

The 15% Life Time discount cannot be applied to an account that already has a corporate, government, or association discount attached to it. If you have an existing discount attached to your account that is less than 15%, contact customer care to have it removed before submitting your Life Time migration form.

does T-Mobile really ask for membership # this time? I did the discount for Freelancer 10% last time and they never asked for membership #.
