Do NOT use this form if: A) You do not know what you are doing, B) If your Citi card doesn't have price protection, or C) If you do not have a reference number from Citi to fill in. Otherwise this form will NOT help you. Also, perhaps someone with good photoshop skills can clean up the image a litte, all I did was scan it as-is.
