Very unhappy. Bought rebate forms with understanding they were valid in my state per
your post only to have all four returned a couple days ago. Contacted Heineken Customer
Service and was told that forms are valid ONLY in NC and VA. What a waste.

I DID call and complain to 1.888.HEINEKEN and they researched, called back and told me the sad news.....
Has anyone in states other than NC or VA had any luck with his/her Heineken rebate/s???

His description is perfectly fine. NO BEER PURCHASE REQUIRED in NC or VA. All other states listed require beer purchase. I have seen these forms in OH, so he is correct. If you bought the required beer, then call and complain. If not, you are probably out of luck.

Well, I guess that you should have read the form BEFORE mailing it and working it out with Alan at that point.

"You need the Heineken forms that the premium cheese, etc goes to P.O. BOX 8019 and the beef to P.O. Box 8021. Those are the Heineken forms that are good in MO and do not require a beer purchase. You just need to find those forms!"
