I can't guarantee my answer is 100% correct, but when I've connected at LHR, I have not had to pass through immigration. The terminal connecting buses stay terminal side although you'll be rescreened. To be sure, contact your airlines.

You have to go in Islamabad head office of embassy or you get better info on there website....
What country issued your passport? If it is India or Pakistan or Nepal you will need a transit visa to change planes. Used to be if you were from one of the Commonwealth countries you didn't need it. My good friend's wife (he is a retired Canadian brigadier and works for UN Peace Keeping forces) is from India and holds Canadian citizenship made the mistake of showing her Indian passport when changing in Heathrow and had a few hassles. Fortunately they both have UN diplomatic passports and she got through but would have had a problem by not showing the Canadian passport first. Schiphol can be the same way. With Mumbai as your origin Frankfort can be really stinky. That applies to just about anywhere in the EU.
The border authority can waive the requirement at THEIR discretion (you don't know until you get there). Check the Embassy or the nearest consulate for better information. Brits have gotten sticky after 7/7 if your face is dark.
